Job summary

Terros Health seeks a dedicated MAT Case Manager for our McDowell Rd Health Center in Phoenix, AZ. This full-time, in-office role focuses on coordinating medication-assisted treatment services, collaborating with a clinical team, and engaging clients to improve outcomes.

The position requires a High School Diploma or GED, with a preferred Bachelor’s or Master’s in behavioral health. Comprehensive benefits, 401(K), and generous PTO available. In-office schedule is Mon–Fri, with flexible options.

Qualifications

• High school diploma or GED required; field-specific study preferred.
• Bachelor’s degree in behavioral health or related field; experience in behavioral health.
• Master’s degree or higher in behavioral health preferred.

Responsibilities

• Coordinate MAT services with the clinical team and clients.
• Conduct outreach and follow-up for services within MAT program.
• Communicate with families, behavioral and medical providers, and other supports.
• Participate in clinical staffing, case conferences, and multi-disciplinary meetings.
• Maintain accurate documentation in the EMR on the same day services are delivered.
