Job summary

Community Health in Rutland, VT is seeking a MAT Care Manager (Registered Nurse) to deliver nursing and care management to patients in MAT services and coordinate with the care team to support health and recovery.

You will work with primary care providers to establish treatment plan goals, perform intakes, and provide follow-up after discharge, ensuring continuity of care and collaboration with behavioral health services.

Qualifications

• Current Vermont RN license; LPN may be considered.
• CPR Certification.
• Prior experience in a nursing position; case management experience in outpatient setting preferred.
• Experience with electronic medical records; able to learn other systems; basic keyboarding and email communication.

Responsibilities

• Provide direct nursing and care management to MAT patients.
• Act as liaison between MAT and medical providers to ensure up-to-date medical needs.
• Support MAT patients in overcoming barriers to care and connecting with community resources.
• Perform MAT intakes and present potential new patients to the MAT team.
• Collaborate with patients in psychiatric and substance abuse programs.
• Follow-up calls and services post discharge for MAT patients from inpatient, outpatient, or residential programs.
