Lamoille Health Partners in Morrisville, VT is seeking a dedicated MAT Counselor to join our integrated Community Health Team. You will provide counseling and support to individuals navigating medication-assisted treatment in a primary care setting.

Qualifications

• Active, unencumbered Vermont license in applicable clinical field.
• Experience with substance use disorder treatment and harm reduction.
• Strong communication and motivational interviewing skills.
• Ability to collaborate in a multidisciplinary team and advocate for patients.

Responsibilities

• Provide individual counseling and crisis intervention within MAT.
• Conduct whole-person assessments and address psychosocial barriers to care.
• Coordinate with primary care and behavioral health teams for care transitions.
• Navigate community resources, housing, and financial assistance for patients.
• Participate in quality improvement and data tracking for MAT program.
