Job summary

Community Health Centers of the Rutland Region in Rutland, VT seeks a Care Manager - RN (will consider LPN) to coordinate follow-up care for high‑risk patients, manage transitions across care settings, and educate patients on medications and care plans.

You will work with care teams to identify barriers, determine visit frequency, and provide support for social determinants of health. A Vermont nursing license and CPR are required, with outpatient case management experience preferred.

Qualifications

• Current Vermont RN/LPN license.
• CPR Certification.
• Prior nursing position experience; case management in outpatient setting preferred.
• Experience with electronic medical records and ability to learn new systems; basic keyboarding and email communication.

Responsibilities

• Provide follow-up care to identified patients based on complexity and social determinants of health.
• Coordinate care with post-discharge concerns or barriers.
• Provide transitional care for risk-stratified patients post-discharge from outpatient or inpatient venues.
• Ensure hospital-discharged patients have education on medications.
• Determine frequency of telephone encounters based on patient need.
• Identify barriers to care and contact appropriate resources.
• May determine that a patient requires a face-to-face visit.
