Qualifications

• BSN is required.
• New York State RN license is required.
• CPR/BLS-HCP certification is required.

Responsibilities

• Act as point person and primary contact for the site.
• Complete MAT intakes by phone or in person.
• Participates in case conference review meetings, department meetings, QI activities, and other relevant meetings.
• Complete Recovery Support Plans.
• Document encounters in EMR with updated progress notes and case records.
• Coordinate care with external agencies and complete consents.
• Provide harm reduction education and Naloxone training.
