Job summary

Qualifications

• Current Vermont RN license; LPN may be considered.
• CPR Certification.
• Prior nursing experience required; prior case management experience in outpatient setting preferred.
• Experience with electronic medical records and ability to learn systems; good keyboarding and email skills.

Responsibilities

• Provide direct nursing and care management services to MAT patients.
• Act as liaison between MAT provider and medical provider to ensure medical needs are met.
• Provide support and coordination to MAT patients to reduce barriers to care and link to resources.
• Conduct post-discharge follow-up calls and services for MAT patients.
• Conduct MAT intakes and present potential new patients to MAT team.
• Collaborate with patients in psychiatric and substance abuse programs.
