Job summary

Qualifications

• RN license in Vermont (active)

• Experience with Electronic Health Records (Athena or similar)

Responsibilities

• Identify patients for Medication Assisted Therapy and help form care plans
• Coordinate care across medical, behavioral health, and social services
• Promote health, activate patients, and support self-management
• Facilitate seamless transitions of care and reduce hospital readmissions
• Participate in staff, supervision, PQI, and clinical meetings
• Act as a liaison to community agencies and healthcare providers
• Maintain collaborative relationships with therapists and medical staff
