Job summary

Stars Behavioral Health Group's Star View Community Services in Torrance, CA is seeking a MAT Bilingual Case Manager (Spanish required). The role focuses on oversight and care management for clients referred to Star View programs in school, home and community settings.

You will conduct assessments, screenings, develop treatment plans, and coordinate services, with supervision for BBS hours where applicable.

Qualifications

• Six years of direct service experience for HS diploma.
• Four years of direct service experience with an associate degree.
• Two years of direct service experience with a bachelor’s degree.
• Valid California Driver's License.

Responsibilities

• Oversee and care manage clients referred to Star View programs.
• Provide case management/linkage, assessments, screenings, and treatment planning.
• Monitor rehabilitation services and coordinate with families and schools.
• Collaborate with clinicians, social workers, and county partners.

The Case Manager will provide services that may include case management/linkage, assessments, screenings, treatment planning/monitoring, and rehabilitation services to the clients and their families.Division/Program Overview:Population Served: Children, Adolescents, and Transition-Age Youth (TAY)Facility & Setting: Field-based / Community settings (schools, homes, and local community facilities)Clinical Services: Comprehensive mental health care, psychiatric evaluation, Evidence-Based Practices (EBP), individual/family therapy, and multidisciplinary care delivered by psychiatrists, psychologists, social workers, and cliniciansLength of Stay: Outpatient/community-based timeline (varies according to individual treatment plan and progress)Skills & Transition: Interagency collaboration and transition support with school administrators, county social workers, probation officers, and case managers.
