Job Title: Behavioral Health Case Manager (Bilingual)
Division/Program: Raising Early Awareness and Creating Hope
Starting Compensation: 24.97-29.96 USD Per Hour
Working Location: San Jose: North San Jose Center (In-Person Position)
Working Hours/Shift: FT, Monday-Friday, AM shift (9 AM-6:30 PM)

Education/Experience:
Applicants must meet any one of the following education and experience requirements:
License Or Certification: Valid California Driver’s License and current automobile insurance and must maintain a driving record that meets the company driver's eligibility policy.
NOTE: This position requires driving from home to home, working with clients in the community, and transporting clients in personal vehicles.
How You Will Make a Difference (job Overview)
This position has primary responsibility for providing oversight and care management to the clients referred to Starlight programs. The Case Manager will provide services that may include case management/linkage, assessments, screenings, treatment planning/monitoring, and rehabilitation services to the clients and their families.
