Job Title: Case Manager | Bilingual Spanish
Division/Program: Raising Early Awareness and Creating Hope
Starting Compensation: 26.62 - 31.94 USD Per Hour
Working Location: San Jose: North San Jose Center (In-Person Position)
Working Hours/Shift: FT, Monday-Friday, AM shift (9 AM-6:30 PM)
This position has primary responsibility for providing oversight and care management to the clients referred to Starlight programs. The Case Manager will provide services that may include case management/linkage, assessments, screenings, treatment planning/monitoring, and rehabilitation services to the clients and their families.
