Job summary

Community Solutions, Inc. is seeking a dedicated Case Manager for the Victorville Probation Office in Victorville, CA, with MAP support in Joshua Tree and Barstow, CA (25% travel).

Responsibilities include intake assessments, individual and group counseling, and aftercare planning within a trauma‑informed framework. The role emphasizes program planning, record‑keeping, and coordination with sub‑contractors to serve justice‑involved individuals.

Qualifications

• Bachelor’s degree in a social science field and one year experience in case management or similar field.
• Excellent oral, written and interpersonal skills desired.
• Experience with Microsoft Office.

Responsibilities

• Conduct client orientation, collect intake data and assess needs; make recommendations to supervisor.
• Develop and document program plan goals with clients and monitor treatment stipulations.
• Develop and implement discharge plans and monitor progress.
• Schedule and document individual counseling sessions addressing client issues and program performance.
• Facilitate group meetings addressing substance abuse and pre-release issues.
• Maintain client files and ensure confidentiality; complete progress and closing summaries.
• Complete all documentation and log entries; supervise weekend staff as needed.
