Job summary

Palos Heights Senior Living is seeking a full-time Licensed Practical Nurse with management experience to lead dementia care and supervise nursing staff. The role includes coordinating admissions, conducting health assessments, and developing care plans in a senior living setting.

The ideal candidate has a nursing license, a bachelor’s degree in a relevant field is preferred, and experience in long-term care. Weekend coverage as Manager-On-Duty may be required.

Qualifications

• Current Licensed Practical Nurse required.
• Caregiver certifications may be beneficial with management experience.
• Minimum 1–2 years of management experience in directing/providing dementia care for seniors preferred.
• Experience in community or long-term care settings and must meet all state requirements.

Schedule: Full-time

In this position, your main responsibilities will include:

• Recruitment, selection, training, supervision, and staffing of team members.
• Implementing resident-centered dementia care.
• Provide nursing care to residents, including but not limited to: Completion of health assessments and resident care plans, discussing changes in resident condition with physicians, introduce interventions to reduce falls based on individual resident needs, etc. Also provide medication administration and help with resident care, as needed
• In conjunction with the community’s multidisciplinary team, coordinate the resident admission process including screening, assessment and care planning.

This position may occasionally fill in on the weekend as a Manager-On-Duty.

Qualifications:

To be successful in this position, we believe that you need the following experiences, strengths, and skills:

• Current Licensed Practical Nurse required.
• Candidates may be considered with caregiver certifications beneficial to the community plus management experience.
• A Bachelor’s degree or higher in Nursing, Social Work, Psychology, or Gerontology preferred.
• Minimum 1-2 years of management experience in directing/providing dementia care for seniors preferred.
• A strong background in patient care assessment, assessing resident behavioral symptoms and developing non-pharmacologic approaches to meeting resident needs.
• Has experience in community or long-term care settings.
• Must meet all state requirements.
