Job summary

Mercy is seeking a Manager for Home Health and Hospice Intake to lead and coordinate intake operations across Mercy Ministries, ensuring consistent and high-quality experiences for patients, families, and referral partners. The role will initially manage Hospice Intake for the East region and drive standardization across the network, with expansion to Home Health Intake in the future.

• RN license active in state of practice is required.
• BSN preferred.
• 3 years of nursing experience.
• Previous leadership/management experience preferred.
• BLS Certification required within 90 days of hire.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.

• Lead Hospice and Home Health Intake operations across Mercy Ministries to ensure a consistent, efficient, high-quality patient experience.
• Initially oversee Hospice Intake for the East region and align Intake services across communities.
• Drive process standardization and optimize resource utilization.
• Monitor operational, quality, and service metrics to meet productivity/financial goals.
• Collaborate with leaders, physicians, referral sources, and caregivers in Hospice, Home Health, and the broader health system.

Qualifications

• Required Education:
  • Graduate from a school of nursing is required.
  • BSN Preferred.
• Experience:
  • 3 year of nursing experience.
  • Previous leadership/management experience preferred.
• Licensure:
  • Active RN (Registered Nurse) license in state of practice is required.
• Certifications:
  • BLS Certification required within 90 days of hire.
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• Other:
  • Demonstrates skill in human relations and communications is required. Must have broad understanding of the patient care process and be able to demonstrate clinical expertise in their area of responsibility.
