Job summary

Mercy in Des Peres, MO is seeking a Manager - Home Health and Hospice Intake to lead and coordinate Hospice and Home Health intake operations across Mercy Ministries, with a focus on ensuring a high-quality patient and family experience.

This role partners with physicians, referral sources, and caregivers, standardizes processes, and supports caregiver development while driving metrics to meet productivity and financial goals.

Qualifications

• Graduate from a school of nursing is required.
• Active RN license in state of practice is required.
• BLS certification required within 90 days of hire.
• 3 years of nursing experience; leadership/management experience preferred.
• Certified Hospice and Palliative Nurse (CHPN) preferred.

Responsibilities

• Lead and coordinate Hospice and Home Health intake operations across Mercy Ministries, with initial focus on Hospice Intake for the East region.
• Drive process standardization and resource optimization for intake services.
• Collaborate with leaders, physicians, referral sources, and caregivers to improve communication and streamline workflows.
• Support caregiver engagement and development to meet quality, productivity, and financial goals.

Responsible for leading and coordinating Hospice and Home Health operations across Mercy Ministries with focus on intake roles, ensuring a consistent, efficient, and high-quality experience for patients, families, referral partners, and caregivers. This role will initially oversee Hospice Intake for the East region and serve as a key leader in the strategic alignment and integration of Intake services across additional Hospice communities, with future expansion to include Home Health Intake. The leader will drive process standardization, support caregiver engagement and development, optimize resource utilization, monitor operational, quality, and service metrics to meet productivity/financial goals. Works collaboratively with leaders, physicians, referral sources, and caregivers across Hospice, Home Health, and the broader health system to improve communication, streamline workflows, and support continued growth and excellence in post-acute care services

Monday - Friday 8a-4:30p

• On call rotation

Qualifications:

• Required Education:
  • Graduate from a school of nursing is required.
    • BSN Preferred.
• Experience:
  • 3 year of nursing experience.
    • Previous leadership/management experience preferred.
• Licensure:
  • Active RN (Registered Nurse) license in state of practice is required.
• Certifications:
  • BLS Certification required within 90 days of hire.
  • Certified Hospice and Palliative Nurse(CHPN) certification preferred.
• Other:
  • Demonstrates skill in human relations and communications is required. Must have broad understanding of the patient care process and be able to demonstrate clinical expertise in their area of responsibility.
