Job summary

Carle Health is seeking a PTA/PT/OT to evaluate and treat patients full time, with a focus on lymphedema care. The role offers a sign-on bonus and a Monday–Friday schedule, 8-hour days. Licensure requirements include IL IDFPR and BLS within 30 days. Compensation will vary with licensure; shift options may differ.

Responsibilities include patient education, care plan design, documentation, and collaboration with the rehab team to ensure safe, effective outcomes. Open to various shifts.

Qualifications

• Bachelor’s Degree in Physical Therapy required.
• Licensed Physical Therapist (PT) – Illinois license (IDFPR) required.
• Basic Life Support (BLS) within 30 days of hire.
• Continuing education in acute stroke care as required.

Responsibilities

• Evaluates and treats patients; designs and updates plan of care;Discharge planning integrated with goals and outcomes.
• Educates patients, families and staff using effective teaching methods.
• Performs examinations and determines PT diagnoses and outcomes.
• Documents in accordance with policy; uses approved examination methods.
• Collaborates with the rehabilitation team and supervises PTA and rehab technicians.
• Communicates treatment progress and discharge recommendations to medical team.
