Job summary

Qualifications

• Bachelor’s Degree in Physical Therapy or related field.
• Licensed Physical Therapist (PT) in good standing.
• BLS certification within 30 days (AHA).
• Participation in continuing education for stroke care as required.

Responsibilities

• Designs and adjusts care plans integrating goals, treatment, outcomes and discharge plan.
• Educates patients and families using effective teaching methods.
• Performs examinations and selects appropriate PT assessment tools.
• Maintains accurate documentation per CTSCP67 policy.
• Supervises PTA and rehab technicians where applicable.
