Job summary

Carle Health in Peoria, IL is seeking a licensed Physical Therapist (PT), Physical Therapist Assistant (PTA), or Occupational Therapist (OT) to join our rehabilitation team. The role emphasizes lymphedema care, patient education, and coordinated care planning.

Sign-on bonus offered; full-time, M-F days; licensure determines compensation. The ideal candidate holds a Bachelor's in Physical Therapy, IL license, BLS within 30 days, and is committed to high-quality patient outcomes and staff

Qualifications

Licensed Physical Therapist (PT) in Illinois (IDFPR)
BLS within 30 days - American Heart Association (AHA)
Lymphedema certification preferred but not required
Bachelor’s Degree in Physical Therapy

Responsibilities

Designs an appropriate plan of care with goals, treatment, outcomes, and discharge plan.
Educates patients, families, and caregivers using effective teaching methods.
Evaluates clinical findings to determine PT diagnoses and outcomes of care.
Performs examinations with reliable methods relevant to the chief complaint and history.
Completes documentation per CTSCP67 and regulatory standards.
Collaborates with patient, family, and care team throughout rehabilitation process.
