Job summary

Carle Health seeks a licensed Physical Therapist (PT) to evaluate and treat patients of all ages, adjusting plans of care as needed. The role serves as a clinical resource and mentor to staff and students, with opportunities for education and leadership within the rehabilitation team.

The position supports full-time hours, offers a sign-on bonus, and requires licensure in Illinois. Lymphedema certification is preferred but not required, with shifts Monday–Friday and 8-hour days.

Qualifications

• Licensed Physical Therapist (PT) - Illinois IDFPR
• Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
• Lymphedema certification preferred but not required
• Attend and satisfactorily complete continuing education regarding acute stroke care

Responsibilities

• Designs an appropriate plan of care that integrates goals, treatment, outcomes and discharge plan.
• Educates patients, family, caregivers, students and other health care providers.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examinations and selects reliable, valid examination methods.
• Produces documentation in accordance with CTSCP67.
• Supervises PTA and rehabilitation technicians and communicates patient status to the medical team.
