Job summary

Carle Health is seeking a Physical Therapist to evaluate and treat patients of all ages, adapting plans of care as needed. Lymphedema certification is preferred but not required. The role offers a sign-on bonus and shifts from Monday to Friday, eight-hour days, with full-time patient care responsibilities.

Successful candidates will supervise PTA and rehab technicians and collaborate with the medical team to ensure effective discharge planning and patient education.

Qualifications

• Education Level: Bachelor’s Degree
• Field of Study: Physical Therapy
• Licenses/Certifications: Licensed Physical Therapist (PT) - IDFPR
• Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
• Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor, while using safe body mechanics.
• Educates patients, families, caregivers, students, and other health care providers using effective teaching methods.
• Designs an appropriate plan of care with goals, treatment, outcomes, and discharge planning.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes.
• Performs examinations with reliable methods relevant to chief complaint and history.
• Produces documentation per CTSCP67 policy.
• Supervises PTA and rehabilitation technicians per regulatory guidelines.
• Communicates patient status, discharge plans, and required equipment to the medical team.
