Job summary

Assisted Home Health & Hospice is hiring LVNs for Winnetka, Reseda and Arleta. Full-time and part-time shifts available. New grads welcome; training provided. You’ll deliver quality, patient-centered home health care with a focus on one patient at a time.

Qualifications

• Graduate of accredited practical (vocational) nursing program.
• Licensed LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.

Responsibilities

• Provide home health nursing services to patients in Winnetka, Reseda and Arleta on shift-based schedules.
• Communicate regularly with the RN Case Manager and physician regarding patient condition changes and needs.
• Assist RN and/or physician in performing specialized procedures and carry out the plan of care.

What you will do:

This is shift work. Ensure quality and safe delivery of home health nursing services. Communicate with RN Case Manager and physician, as appropriate, on a regular basis regarding changes in the patient's condition and/or needs. You will assist the RN and / or physician in performing specialized procedures and duties. Assist the RN in carrying out the plan of care.
