PIH Health is seeking a Patient Care Coordinator, LVN for Home Health in LA/OC County. The role supports the interdisciplinary team by coordinating physician orders, relaying lab results, and scheduling patient visits to ensure seamless care delivery.

The LVN will collaborate with nursing leadership, address issues promptly, and act as the Home Health representative with patients, families, physicians, and community partners.

Formación

• Current California LVN license.
• Minimum 1 year professional LVN experience.
• Current CPR certification.
• Valid California Driver’s License and auto insurance.

Responsabilidades

• Obtain physician orders and communicate patient lab results to physicians and RN case managers.
• Schedule patient visits and coordinate care with the interdisciplinary team.
• Serve as Home Health representative when engaging with patients, families, physicians, and the community.
• Troubleshoot and resolve issues with nursing leadership.
