PIH Health is seeking an Intake Coordinator LVN for Home Health, 8am - 4:30pm, to work under the Clinical Director. Responsibilities include coordinating intake, scheduling services, and ensuring smooth flow of clinical information to improve patient outcomes.

QUALIFICATIONS include a California LVN license, minimum one year of LVN experience (home health or hospice preferred), CPR, and a valid California driver's license with insurance preferred.

Formación

• Minimum one year experience as a LVN.
• Home Health or Hospice experience preferred
• Current CPR card
• Current California driver’s license and valid auto insurance preferred
