Qualifications

• Graduate of an accredited school of Nursing.
• Current Texas LVN licensure.
• Basic Life Support from an AHA‑approved program.
• 3 years of acute or post‑acute care experience.
• Experience caring for chronic disease patients preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Coordinate activities to facilitate patient care and promote disease management, wellness services, appointments, scheduling, referrals, tests, and follow‑ups.
• Serve as a point of contact and information resource for patients, families, care team, payers and community resources.
• Facilitate patient access to resources and appropriate utilization of systems and processes.
• Ensure compliance with quality standards to meet outcome targets for patient populations.
• Perform duties in telephonic, in‑clinic and home settings as needed.
