An established home health provider is seeking a Licensed Vocational Nurse (LVN) Care Coordinator to support patient care coordination, clinical oversight, and quality assurance. This role works closely with clinical staff and leadership to promote continuity of care, maintain regulatory compliance, and ensure patients receive appropriate, high-quality services.
Although primarily office-based, the position includes occasional patient visits to support urgent clinical coverage and staffing needs.
This is primarily an office-based role, and routine field visits are not expected. However, LVN Care Coordinators may be assigned patients within their local county when urgent clinical coverage is needed.
When temporary coverage is required, the coordinator may continue caring for the assigned patient until discharge or until appropriate coverage becomes available. Unlike regular field nurses, this position does not typically require routine travel within a designated mileage radius.
Additional requirements:
If you are an experienced LVN with a background in home health case management who enjoys coordinating care, supporting clinical teams, and improving patient outcomes, we invite you to apply.
This opportunity offers a primarily office-based schedule with clinical responsibilities that help ensure patients receive consistent, high-quality care when additional support is needed.
