Job summary

PIH Health is seeking a Licensed Vocational Nurse (LVN) for Home Health in the LA/Orange County area. This part-time role runs 8:00 am to 4:30 pm, delivering direct patient care in a home setting under the supervision of a Registered Nurse.

The LVN will provide nursing care within the scope of LVN practice, coordinate with the care team, document in the electronic health record, and support Patients First initiative while ensuring patient comfort and safety.

Qualifications

• Current California license as LVN in good standing.
• Minimum of one year nursing experience, preferably in an acute care facility.
• Current CPR card.
• Current California driver's license and valid auto insurance.

Responsibilities

• Provide direct patient care in the patient’s home under RN supervision.
• Practice within LVN scope and uphold PIH Health Patients First vision.
• Operate within home health standards and use an electronic health record.
• Adapt care to patient and family needs.
