Job summary

PIH Health is seeking a Licensed Vocational Nurse to serve as Intake Coordinator for Home Health. Under supervision of the Clinical Director, you will coordinate intake information, schedule services, and ensure smooth flow of clinical data to support timely patient care.

This part-time role runs 8:00 a.m. to 4:30 p.m., requires at least one year of LVN experience (home health or hospice preferred), CPR, and a California driver’s license. Strong communication and computer skills are essential.

Qualifications

• Minimum one year experience as a LVN.
• Current CPR card
• Current California driver’s license and valid auto insurance preferred

Responsibilities

• Coordinate intake of information and scheduling of services to ensure efficient workflow.
