Job summary

Sweetwater Care in Long Beach, CA is seeking an experienced LVN/LPN to provide nursing care in a long-term setting. The role requires a current LVN/LPN license, BLS/CPR certification, and familiarity with PointClickCare charting to document patient needs accurately.

The nurse will participate in rounds with physicians, document patient condition changes, and follow established procedures for charting, narcotic counting, and incident reporting while adhering to center policies and state/local

Qualifications

• Long Term Care experience.
• Point Click Care charting experience.
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
