Job summary

Sweetwater Care is seeking a licensed LVN in California with nursing management experience to oversee resident care and support staff in a long-term care setting. The role emphasizes adherence to physician orders, quality improvement initiatives, and efficient charting using PointClickCare.

The incumbent will coordinate patient placement, educate families, and ensure timely administration of medications and treatments in alignment with policy; rounds with physicians may be required.

Qualifications

• Licensed LVN in California; compact license required for a Compact state.
• One year of nursing management experience preferred.
• Experience in long-term care.
• Experience with PointClickCare charting.

Responsibilities

• Provide nursing care per standards and physician orders.
• Implement Quality Improvement Program.
• Utilize Quality Life Manual for patient placement and monitoring.
• Notify departments of patient changes (room changes, LOA, admissions, discharges).
• Educate patients and families.
• Administer medications timely per facility policy.
• Administer treatments timely per facility policy.
• Transmit and execute physician orders as allowed by Nurse Practice Act.
• Make rounds with physicians as needed.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
