Job summary

Sweetwater Care in California is seeking an experienced LVN to assume nursing management responsibilities in a long-term care setting. The role focuses on delivering quality patient care within state guidelines and facility policies.

You will participate in infection control, quality improvement, and patient education, oversee medication administration, and ensure accurate physician orders are carried out. California LVN license and compact license where required are essential.

Qualifications

• Licensed LVN in California is required.
• One year of nursing management experience preferred.
• Long-term care experience required.
• PointClickCare charting experience preferred.

Responsibilities

• Demonstrates knowledge and uses Infection Control Manual.
• Supports Quality Improvement Program.
• Identifies, places, and monitors patients in appropriate programs.
• Notifies departments of patient changes, e.g., room changes, LOA, new admits, discharge.
• Conducts family and patient education.
• Administers medications and treatments per facility policy.
• Receives, transcribes, and carries out physician orders per Nurse Practice Act where allowed.
• Mops rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
