Job summary

Sweetwater Care in Hawthorne, California, is seeking an LVN/LPN to provide nursing care within scope of practice, following physician orders and center policies. The role requires Long Term Care experience, PointClickCare charting, and an active California LVN/LPN license with current BLS/CPR.

Nursing management experience is preferred but not required. This position also uses the Quality Life Manual for patient placement and monitoring, and requires timely documentation of room changes, LOA,

Qualifications

• Long Term Care experience required.
• PointClickCare charting experience required.
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification required or willingness to acquire one.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
