AltaMed Health Services is hiring an LVN Case Manager to coordinate daily patient care for hospital and SNF populations, including ambulatory care and CCS case identification. The role involves telephonic coordination with an interdisciplinary team and social work, ambulatory care, and hospital staff.

Ideal candidates hold an accredited LVN program diploma and an active LVN license in California, with bilingual English/Spanish preferred.

Qualifications

• Graduation from an accredited LVN nursing program.
• Current valid License as a Licensed Vocational Nurse (California).
• Bilingual in English and Spanish preferred.

Responsibilities

• The LVN Case Manager provides daily care coordination with concurrent review for patients in hospital or SNF, telephonically.
• Case manage patients admitted to hospital or SNF and those enrolled in ambulatory case management.
• Identify CCS cases, handle transfers, and coordinate with interdisciplinary care teams.

BLS certification or higher
