Job summary

Pacific Health Group is seeking a Licensed Vocational Nurse (LVN) to join the Enhanced Care Management team in Hiring County. In this hybrid role, you will support high-risk Medi-Cal members with care coordination, education, and medication reconciliation under RN supervision.

You will perform health assessments, document observations, and collaborate with nurses, community health workers, and providers to improve outcomes and reduce barriers to care. Current CA LVN license required.

Qualifications

• Current and unrestricted California LVN license.
• Minimum 1 year of clinical healthcare experience (healthcare, managed care, home health, public health, behavioral health, post-acute care).
• Experience working with underserved or vulnerable populations preferred.
• Strong understanding of care coordination and chronic disease management.
• Excellent communication, organizational, and interpersonal skills.
• Must reside in or near Hiring County and be able to travel up to 60–70%.

Responsibilities

• Support ECM care coordination activities and member assessments.
• Conduct health screenings and document vital signs and observations.
• Assist with medication reconciliation and education under RN supervision.
• Engage in field visits, outreach, and collaborations with providers and organizations.
• Maintain accurate documentation in EHR and care management systems.
