Job summary

Sweetwater Care in Malibu, CA is seeking an experienced LVN/LPN to provide nursing care within scope of practice, following physician orders and center policies, and ensuring standards of care are met. Responsibilities include charting, incident reporting, narcotic counting, documenting patient condition changes, rounds with physicians, and coordinating admissions/discharges.

BLS/CPR certification is required or willingness to obtain; nursing management experience is a plus; Sweetwater Care is

Qualifications

• Long Term Care experience required.
• Experience with PointClickCare charting.
• Active LVN/LPN license in practicing state.
• BLS/CPR certification or willingness to obtain.
• Nursing management experience preferred.

Responsibilities

• Follows established procedures for charting and incident reporting.
• Adheres to narcotic counting, pharmacy orders, and restraint reduction procedures.
• Documents patient condition changes (nutrition, infections, skin, weight, etc.).
• Uses Quality Life Manual for patient placement and monitoring.
• Documents room changes, LOA, new admits, or discharges; coordinates with departments.
• Mediates rounds with physicians when necessary.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
