Job summary

Harbor Health in Austin, TX is seeking Care Coordinators (LVN) to support clinical inbox management, medication refills, and care plan coordination. You will liaise with providers, members, and staff, applying LVN judgment and Harbor Health protocols to ensure timely responses and accurate documentation.

Qualifications

• Active Texas LVN license or compact state privilege.
• 3+ years of experience in a medical office or healthcare setting.
• 2+ years of LVN experience in ambulatory/ primary care.

Responsibilities

• Manage and prioritize provider incoming clinical communications.
• Route, escalate, and document messages in the EHR.
• Support medication refill review and care plan coordination.
• Educate members on medications and care plans within LVN scope.
• Identify safety concerns and escalate per protocol.
