Job summary

The Elizabeth Hospice is seeking a Patient Care Coordinator, LVN, to support the Clinical Services Team. The role handles clerical tasks, medical record maintenance, IDT prep, and coordination of physician orders to ensure timely care from admission through discharge.

Responsibilities include managing patient communications, data reports, and liaising with physicians, nurses, and families. LVN license preferred; in-office schedule Mon-Fri, no weekends or on-call.

Qualifications

• High School diploma or equivalent; 3-4 years in healthcare field.
• LVN license preferred.
• Proficiency in medical terminology and HIPAA regulations.
• Strong typing and MS Office skills; adaptable to new software.

Responsibilities

• Generates and monitors staff data integrity reports for staff compliance.
• Processes physician recertifications and DNR/POLST orders for timeliness.
• Maintains and updates clinical records from admission to discharge.
• Prepares correspondence and reports for NP/MD visits and IDT meetings.
• Interacts with patients, families, and staff with compassionate communication.
