Harbor Health in Austin, TX is seeking Care Coordinators (LVN) to join our clinic operations team. You will apply LVN clinical judgment and Harbor Health-approved protocols to manage and organize incoming clinical communications, ensure timely responses, and facilitate effective communication between providers and members.

Formación

• Active Texas LVN license or compact state privilege.
• 3+ years in a medical office or healthcare setting.
• 2+ years LVN experience in ambulatory/primary care.
• Strong organizational skills and clinical terminology knowledge.
• Proficient with Microsoft Office and EHR systems.

Responsabilidades

• Clinical Inbox & communication management with prioritization and routing.
• LVN clinical support for med refills and care plan coordination.
• Educate members on medications and follow-up instructions.
• Escalate safety concerns per Harbor Health protocol.
• Support process improvement in inbox workflows.

Conocimientos

Active Texas LVN license

Ambulatory care experience

EHR experience (Athena)
