Elara Caring is seeking an LVN Clinical Coordinator to support patients, caregivers, and field teams in a primarily office setting. The role emphasizes coordinating care, processing clinical workflows, and maintaining regulatory compliance while protecting patient privacy.

You will report to Branch Director/Clinical Manager and contribute to smooth operation, with potential for occasional field visits and direct patient care as needed. LVN license in Texas and home health experience are required.

Qualifications

• Graduate from an accredited school of nursing.
• Current, unrestricted LVN license valid in Texas.
• At least 1 year of nursing experience in home health.
• Strong organization, communication, and attention to detail.

Responsibilities

• Reviewing and processing Homecare Homebase coordination notes.
• Handling administrative and clinical workflows accurately and on time.
• Enter physician orders for review and approval.
• Maintain At-Risk Registry and support patient follow-up.
• Assist with chart audits and HIPAA compliance.
• Support branch with occasional field visits and direct patient care as needed.
