Job summary

Molina Healthcare is seeking a Care Coordination Specialist within Illinois to support LTSS and care management for high-need members. You will collaborate with multidisciplinary teams, develop and monitor care plans, and facilitate waiver enrollment while navigating regulatory timelines.

Travel 25-40% is expected based on state requirements and contracts. Strong communication, problem-solving, and ability to work independently in diverse settings are essential.

Qualifications

• 2+ years healthcare experience, 1+ year LTSS/care management or equivalent
• License/certification optional per state contract
• Valid driver’s license and transportation for travel
• Experience with community resources and diverse populations

Responsibilities

• Complete member assessments within timelines, including home visits as required
• Coordinate waiver enrollment/disenrollment
• Develop and monitor care plans with members and providers
• Educate and motivate members using motivational interviewing
• Promote integrated services across LTSS and community resources
• Identify barriers to care and address psycho/social/medical obstacles
