Job summary

Molina Healthcare is seeking a care management professional to support LTSS and care coordination for high-need members in Illinois. You will collaborate with multidisciplinary teams to plan, implement and monitor care across the continuum.

Responsibilities include comprehensive assessments, waiver services coordination, and ensuring timely, effective member outcomes. A valid license and travel readiness are required; the role supports diverse populations and settings.

Qualifications

• 2+ years in health care with at least 1 year in care management or LTSS.
• Active and unrestricted professional license (LCSW/LMSW/APSW/LPC/LPCC/LMFT/PhD or PsyD).
• Valid driver's license and reliable transportation for travel.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and care teams.
• Monitor care plan effectiveness and document interventions and outcomes.
• Promote integration of services for LTSS and behavioral health.
