Job summary

Molina Healthcare is seeking a Care Manager in Illinois to support LTSS care coordination. You will collaborate with a multidisciplinary team to ensure timely assessments, comprehensive care plans, and effective navigation of waiver services.

The role requires at least 2 years in healthcare, knowledge of community resources, and strong communication. Travel 25-40% may be required; licensure or certification may be needed per state contract.

Qualifications

• Minimum 2 years healthcare experience with LTSS or care management background.
• Valid driver’s license, reliable transportation, and auto insurance for travel.
• Ability to work independently with minimal supervision and manage multiple projects.
• Strong verbal and written communication, and knowledge of community resources.
• Licensure/certification required only if state contract mandates it.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, families, and providers.
• Monitor care plans and document interventions and outcomes.
• Promote integration of LTSS with behavioral health and community resources.
• Assess medical necessity and authorize waiver services as appropriate.
• Collaborate with interdisciplinary care teams for approvals.
• Educate and motivate members using motivational interviewing and clinical guidelines.
• Identify barriers to care and coordinate psycho/social and medical support.
• Identify incidents and develop prevention plans to protect member welfare.
• Travel 25-40% as required by state/contractual requirements.
