Qualifications

• At least 2 years of health care experience, including LTSS.
• 1 year experience in care management or equivalent.
• Valid driver's license and reliable transportation for travel.
• Demonstrated knowledge of community resources.
• Ability to work independently and handle multiple projects.
• Strong verbal and written communication skills.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and other professionals.
• Performs ongoing monitoring of care plan to evaluate effectiveness and goals.
• Promotes integration of LTSS, behavioral health, and community resources to enhance care continuity.
• Assesses medical necessity and authorizes waiver services.
• Facilitates ICT meetings for approval/denial of services.
• Uses motivational interviewing to educate and motivate members.
• Identifies barriers to care and coordinates resources for psycho/social, financial, and medical obstacles.
• Travel 25-40% may be required.
