Job summary

Molina Healthcare seeks a care management professional to coordinate long-term services and supports (LTSS) for high-need members. You will work with a multidisciplinary team to plan and monitor care, ensuring quality and cost-effective outcomes.

The role involves coordinating waiver services, educating members and caregivers, and navigating benefits within regulatory timelines. Travel to local sites may be required based on state contracts and member needs.

Qualifications

• At least 2 years of health care experience with LTSS exposure.
• Clinical licensure/certification required only if mandated by state contract or licensing rules.
• In some states, a health-care-related bachelor’s degree may be required.
• Valid, unrestricted driver’s license with reliable transportation.
• Proven knowledge of community resources.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor care plan effectiveness and document progress toward goals.
• Promote integration of LTSS, behavioral health, and community resources.
• Collaborate with licensed care managers and leadership as needed.
• Travel 25–40% locally as required by state contracts.
