Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members and care team.
• Monitors care plan effectiveness and documents progress.
• Coordinates services across behavioral health and LTSS to ensure continuity of care.
• Assesses medical necessity and authorizes waiver services.
• Advocates funding sources and evaluates benefits.
• Leads ICT meetings for service approvals and collaborates with teams.
• Address barriers to care and assist with psycho/social and medical obstacles.
• Estimates travel requirements (25-40%) as needed.
