Job summary

Molina Healthcare is seeking a Care Manager to support LTSS-focused activities and coordinate integrated member care. You will collaborate with a multidisciplinary team to ensure progress toward desired outcomes and contribute to strategies for high-quality, cost-effective care.

Responsibilities include comprehensive assessments, waiver services management, and developing dynamic care plans with members, caregivers, physicians, and other health professionals.

Qualifications

• 2+ years healthcare experience, LTSS exposure preferred.
• License/certification required only if state contract mandates it.
• Valid driver's license and reliable transportation for travel requirements.
• Experience with diverse populations and multidisciplinary teams.

Responsibilities

• Perform comprehensive member assessments within timelines, including home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and monitor care plans with members and care network.
• Coordinate services to ensure quality, cost-effective member care.
• Support ICT meetings for approval/denial of services.
• Use motivational interviewing to educate and motivate members.
• Travel 25–40% as required by state/contracts.
