Job summary

Molina Healthcare is seeking a Care Coordinator to support care management and LTSS activities. You will collaborate with a multidisciplinary team to coordinate member care across the continuum and help members progress toward outcomes.

Responsibilities include assessments, waiver enrollment, care plan development, monitoring, and promoting integration of services, with 25–40% local travel. A valid license and LTSS experience are preferred.

Qualifications

• At least 2 years of health care experience incl. LTSS and care management.
• Valid and unrestricted driver's license with reliable transportation.
• Ability to work independently and with diverse populations.
• Knowledge of community resources and referral networks.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and others.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Promotes integration of LTSS and behavioral health services and community resources.
• Assesses medical necessity and authorizes waiver services as appropriate.
• Facilitates interdisciplinary care team meetings for approvals/denials.
• Uses motivational interviewing and Molina clinical guideposts to educate and motivate change.
• Assesses barriers to care and provides coordination to address psycho/social, financial, and medical obstacles.
