Qualifikationen

• 2+ years healthcare experience, LTSS experience preferred.
• Active driver’s license and reliable transportation.
• Proficiency with Microsoft Office and online portals.
• Clinical licensure/certification as required by state contract/regulation.

Aufgaben

• Complete comprehensive member assessments within regulated timelines and via home visits as needed.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with the member, caregivers, physicians, and support network.
