Qualifications

• At least 2 years of health care experience, LTSS exposure preferred.
• Care management experience or equivalent with strong coordination.
• Valid driver's license and reliable transportation.
• Proficiency with Microsoft Office and online portals.

Responsabilités

• Completes comprehensive member assessments within timelines and may require home visits.
• Facilitates waiver enrollment/disenrollment processes.
• Develops and monitors care plans with members, families, and providers.
• Coordinates with ICT and other teams to ensure integrated care.
• Assesses medical necessity and supports funding/benefit decisions.
• Travel up to 25–40% as required by state contracts.
