Job summary

Molina Healthcare is seeking a qualified care management professional to support LTSS members. You will assess needs, develop and monitor care plans, coordinate waiver services, and collaborate with a multidisciplinary team to ensure quality, cost-effective member care.

Required licensure for clinical social work and health-care experience with disabilities or chronic conditions is essential. Travel 25-40% may be required depending on state contracts and client needs.

Qualifications

• 2+ years in health care; 1+ year in care management or LTSS settings
• Licensed clinical social work or equivalent licensure in state
• Bachelor's degree in health care field may be required in some states

Responsibilities

• Perform comprehensive member assessments within timelines, including home visits as required.
• Coordinate waiver enrollment/disenrollment processes and care plans.
• Monitor and adjust care plans; document interventions and outcomes.
• Collaborate with interdisciplinary team to integrate behavioral health with LTSS.
