Molina Healthcare, Inc. seeks a Care Management professional to support LTSS activities and coordinate member care across the continuum. You will work with a multidisciplinary team to ensure quality and cost-effective care for high-need members.

Responsibilities include assessments, waiver processing, care planning, and monitoring outcomes, with 25–40% travel to various counties. Requires 2+ years of health care experience, driving license, and strong communication skills.

Formación

• At least 2 years of health care experience with LTSS, care management, or related field.
• Active driver's license, reliable transportation, and travel willingness.
• Strong communication and documentation skills in clinical settings.
• Experience with community resources and coordinating services.
• Ability to work independently and manage multiple tasks.
• Proficiency with Microsoft Office and online portals.

Responsabilidades

• Complete comprehensive member assessments within timelines, including home visits.
• Facilitate waiver enrollment/disenrollment and develop care plans with the team.
• Monitor care plans, document progress, and adjust as needed.
• Coordinate LTSS and behavioral health services with providers and families.
• Assess medical necessity, approve appropriate waiver services, and manage ICT meetings.
• Identify barriers and provide coordinated support for psycho/social and financial needs.
