Job summary

Molina Healthcare seeks a qualified care management professional to coordinate LTSS for high-need members. You will conduct comprehensive assessments, develop and monitor care plans, and collaborate with the care team including physicians, caregivers and support networks.

Travel may be required regionally to support waiver services and enrollment activities. The role emphasizes proactive problem solving, strong communication, and the ability to work across diverse settings within regulated

Qualifications

• At least 2 years in health care with care management or LTSS experience.
• Active licensure in a social work or related field as listed above.
• Valid driver's license and reliable transportation for travel.
• Ability to work independently and manage multiple tasks.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person visits.
• Develop and implement care plans with members, caregivers and professionals.
• Monitor and adjust care plans based on progress and barriers.
• Coordinate LTSS and waiver services and enrollments/disenrollments.
• Facilitate ICT meetings for approval/denial of services and team collaboration.
• Use motivational interviewing to educate and motivate members.
